Photo, Video, and Audio Release Forms for Lesley University Events

All students, alumni, faculty, and staff participating in any photography or video work for Lesley University must submit a signed release form prior to the event or shoot. This ensures your permission for Lesley University to use your image, audio, or visual works, as well as your written or verbal statements, for promotional purposes in publications, advertisements, videos, websites, social media, and other official formats.

How to Complete the Process:

• Review the purpose and type of each release form below.

• Download and complete the appropriate form before the scheduled photo or video shoot.

• Submit your signed form to the Office of Marketing and Communications via marketingteam@live.lesley.edu.

Important Notes:

• Signed releases give Lesley University permission to use your photo/audio/video for any promotional use, now or in the future. No compensation is provided.

• You waive the right to approve use or review materials before publication.

• Footage and images are for University use only and will not be sold to third parties.

• For minors, a parent or guardian’s signature is required.

• All information provided in the forms is kept confidential and protected under university policy.
