Creating a Dynamic Forms Account for Non-Lesley Users

If a user does not have an active Lesley email address, an account will need to be manually created to fill out a form. This can be done by visiting https://dynamicforms.ngwebsolutions.com or by clicking the link provided to the form being filled out. The login page is pictured below.

Click ‘Create New Account’:

 Enter the account information on the page below and click ‘Create Account’:

After clicking ‘Create Account’, the below message will be displayed and an email will be sent:

Click ‘Confirm Email’ in the email sent from Dynamic Forms:

After clicking ‘Confirm Email’, the below message will display on the Dynamic Forms site. Click ‘Log in to Dynamic Forms’ to return to the original login page and log in to Dynamic Forms:
