Electronic Forms for Advisors and Faculty to Submit to the Registrar

These forms are to be submitted by Lesley University advisors or faculty members ONLY. Do not provide access to these forms to anyone outside of these groups. 

If you are looking for Registrar forms for students (e.g. the Change of Address form), those may be found on the Registrar’s section of the Support Hub knowledge base. If you have questions regarding these electronic forms or their processes, please contact the Registrar’s Office at registrar@lesley.edu or 617-349-8740.  

Advisor Forms 

The Program Change Form should only be completed by the Student’s Advisor after discussing options with the Student. If you are unsure of any of the codes necessary for completion of this form, please consult Informer, Colleague, or the relevant Program Director. Be sure to use the student’s @lesley.edu email when providing their contact information. For an overview of the Program Change Form workflow, click here. 

The Course Substitution OR Waiver should be completed by the Student’s Advisor after discussion with the Student and after any internal approval processes have been completed. The Program Chair or Director will be required to electronically approve the request before the Registrar will process it. In necessary cases, a Program Chair or LH Success Coach may submit the request themselves. For an overview of the Course Substitution or Waiver Form, click here. 

Faculty Forms 

The Grade Change Form must only be completed by the instructional faculty member who assigned the original grade. For policies regarding grade changes (including information about deadlines), please consult the University catalog. For an overview of the Grade Change Form workflow, click here. 

The Independent Study Course Contract should be initiated by the faculty member who will be supervising the student. Be sure to use the student’s @lesley.edu email when providing their contact information. Please speak with your Associate Provost if you have any questions.
