What if I changed my mind or made a mistake on my health insurance form?

To edit/update information:

1. Go to www.gallagherstudent.com/lesley.
2. Under "Profile," enter your email address and press the button labeled "log in." Follow the login instructions.  
3. Navigate to "Account Details."
4. Select "Edit Waiver." 
   1. Select the form you want to edit.
   2. Update the form as needed.
   3. Click "Update & Submit." 

You can edit your waiver up to 5 times.

You may not edit your form after August 1.

If you’ve changed your mind and/or submitted the wrong form:

1. Go to www.gallagherstudent.com/lesley.
2. Under "Profile," enter your email address and press the button labeled "log in." Follow the login instructions. 
3. Navigate to "Account Details."
4. Click "Click here to rescind your waiver." 
5. Click "Rescind My Waiver."

Once the waiver is rescinded, this action cannot be reversed.

You may not edit your form after August 1.
