Using the Lesley Medical Portal (Medicat)

Accessing the Medical Portal

The Lesley Medical Portal is powered by Medicat and can be accessed at https://lesley.medicatconnect.com.

 

You will log in using single sign-on (SSO)—the same username and password as your Lesley email.

 

Access to the portal is available once you are a registered student. If you are not yet registered, you will not be able to log in, but you may begin gathering your health documents in advance for upload once access is granted.

Portal Overview

  • Red dots or asterisks indicate missing documents or unread secure messages.
  • The portal interface may look slightly different on a phone versus a computer.
  • Check your portal annually to ensure all information is current.

Instructions for using the Medical Portal


1. Annual TB Risk Assessment Questionnaire

This questionnaire must be completed every year.

  • Access the link on the portal home page or under “Forms.”
  • Complete all questions and submit.

 

2. Immunizations

  • Click “Immunizations.”
  • The “Missing Immunizations” box shows which vaccines or documents are still required.
  • For each required vaccine:
    • Enter the date(s) you received each dose and submit.
    • You will be directed to the “Uploads” section.
    • Upload proof of immunization by clicking “Select File” and choosing the document from your computer.

 

3. Immunization Record (Summary)

  • Upload a one-page immunization summary from your healthcare provider.
  • Go to “Uploads,” select “Immunization Record,” click “Select File,” and upload the document.

 

4. Vaccines Not Listed on the Summary

If you received a required vaccine that is not included on your immunization summary:

  • Enter the date the vaccine was administered.
  • Upload documentation showing the vaccine name and administration date.
  • The document must state that the vaccine was administered.
  • Receipts for vaccine purchase are not acceptable.
  • Upload this documentation as an “Immunization Record.”

 

5. Titers (Proof of Immunity)

If submitting blood test results:

  • Go to “Uploads.”
  • Select the label for the specific vaccine (e.g., Hepatitis B Titer).
  • Click “Select File” and upload the document.

 

6. Medical or Religious Exemptions

Exemption forms are available under “Forms” or “Uploads.” Forms must be downloaded, completed, and uploaded.

 

a. Medical Exemption

  • Complete the exemption form, checking all applicable boxes, and sign and date it.
  • Your healthcare provider must complete their section, sign, and date the form.
  • The form must specify the vaccine(s) for which exemption is requested.
  • A provider letter must accompany the form and include:
    • The medical condition preventing vaccination
    • Provider letterhead
    • Provider signature and date
    • Your name and date of birth
  • Upload the completed documents under “Medical Waiver.”
  • After uploading, email SHS@lesley.edu so your request can be submitted for review.
  • You will be notified of the decision.

 

b. Religious Exemption

  • Complete the exemption form, indicating the vaccine(s) requested for exemption.
  • Include a personal statement explaining how your religious beliefs prevent vaccination.
  • The statement must be signed and dated and include your name and date of birth.
  • Upload the documents under “Religious Waiver.”
  • After uploading, email SHS@lesley.edu so your request can be submitted for review.
  • You will be notified of the decision.

 

7. Document Requirements

All uploaded documents must:

  • Be in English
  • Clearly display your name and date of birth
