This guide explains how to complete the Lesley University Event Request Form. This form is required to initiate the planning, routing, and approval process for university-hosted events.
Step 1: Routing Selection
Before accessing the main request form, you must identify the appropriate administrator for approval routing.
Navigate to the Instructions page.
Locate the dropdown menu labeled Vice Provost or Academic Cabinet Member.
Select the relevant name from the list.
Note: The names are formatted as Description: Last Name, First Name.
Click Continue to proceed to the Event Request Form.

Step 2: Completing the Event Request Form
The form is divided into sections covering logistics, budgeting, and marketing. Required fields are marked with a red asterisk (*).
1. Contact & Basic Logistics
Contact Info: Verify your First Name, Last Name, Lesley ID #, and Email.
Event Details: Enter the Event Name, Date, and both Start and End Times.
Location: Identify the Event Sponsor/Host and your Preferred Location on campus.
2. Support & Logistics Needs
Use the dropdown menus to indicate what resources you require:
Catering & AV: Specify if you need food services or audio/visual equipment.
Events Team Support: Select the level of assistance needed from the University Events team.
Audience: Define the Intended Audience (e.g., Students, Faculty, Public) and provide the Intended Audience Size.
3. Purpose & Budgeting
Purpose: In the text box, provide a detailed description of why the event is being held.
Financials: * Indicate if there is a Budgeted Expense.
Enter the Budgeted Amount.
Provide the Budget Code/GL Number to ensure proper fund allocation.
4. Marketing & Strategic Alignment
If your event requires promotion, you must complete the final section:
Strategic Area: Select which university pillar this event supports (e.g., Academic Focus, Student Life).
Call to Action: Describe who the audience is and what you want them to do (e.g., "Register by Friday").
Materials: List the specific items you need for promotion (e.g., social media graphics, flyers, or email blasts).

Submission
Once you have completed the form to the best of your ability, click Submit. The request will be automatically routed to the Cabinet Member you selected in Step 1 for review.
