"Invalid ID or Password" error accessing Dynamic Forms

If your login to Dynamic Forms has timed out, you will end up on the general Dynamic Forms login page, NOT the Lesley Dynamic Forms login page.  Your Lesley credentials will not work.  If you attempt to log into this general site it will give you the Invalid ID or Password message below.  Make sure you go to the Lesley Dynamic Forms login page.  The URLs are different and are shown below.  The Lesley Dynamic Forms login page has the Lesley logo.
